Site Director Celene Marin stated that a child in Room #6 was scheduled to transition to Room #7. She will transition this child on Monday, 10/30/23. Based on interviews with staff it was determined that Room #6 was out of ratio with one teacher and an aide to 16 children.

The director states that she will conduct a staff meeting to train staff on the personal rights of children in care. The director also states that she will notify parents timely, within the next business day of notification of a positve Covid case. The director will send LPA a copy of the meeting agenda and staff sign in sheets by POC due date of 3/31/22. Based on interviews conducted by LPA and record review the director was notified that C1 was diagnosed positive for Covid-19 on 12/22/21 but failed to notify parents of the Covid exposure. C1 was last in the facility prior to diagnosis on 12/20/21. This poses a potential health and safety risk to the children in care due to the COVID-19 pandemic.

The surface of the outdoor activity space shall be maintained: Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard.
