101438.1(d) · Infant Care General Sanitation 101438.1 (d) Objects used by infants that are mouthed, shall be washed and disinfected at least daily, or more often if necessary. Such objects shall include, but not be limited to, toys and blankets.

Director states a deep cleaning is scheduled for 8/29/25. Director will implement more regular deep cleaning. A monthly deep cleaning will be added to the maintenance plan. Based on observation and interview the facility did not ensure that the outdoor toys for the infants were not kept clean and sanitary which poses a potential Health, Safety or Personal Rights risks to persons in care.

101430(a)(3)(A)(4)(a) · Infant Care Activities (A) Staff shall place infants up to 12-months of age on their backs for sleeping. (4) Infants with an Individual Infant Sleeping Plan [LIC 9227 (3/20)] that have Section C of the form completed and signed by an authorized representative shall be placed on their back when first laid down to sleep. In the event the infant changes position, the infant may remain in the alternative position. (a) Upon staff observation that the infant is capable of rolling from their back to their stomach and stomach to their back, the provider shall fill out Section D of the Individual Infant Sleeping Plan [LIC 9227 (3/20)], notify the authorized representative, and obtain the authorized representative's signature on the individual sleeping plan no later than the next business day following observation.

Director Andrea D'agostino stated she will coach staff on proper reporting of "ouchies" to parents and submit a sign-in/ agenda to LPA by POC due date of 11/4/22. Based on interview, observation, and record review this requirement was no met as a child was scratched and parent was not notified until she inquired about the injury at pick up. An ouch report was provided afterwards. This poses a potential health, safety, or personal rights risk to children in care.

101438.1(c)(2) · 101438.1(c)(2) Infant Care General Sanitation: Carpeted floors and large throw rugs that cannot be washed shall be vacuumed at least daily and cleaned at least every six months, or more often if necessary.

Director stated she will add additional carpet care to cleaning services to include deep cleaning with janitor who cleans 3 times a week and also, increase frequency of cleaning by teaching staff. She stated she will request service ans submit proof within 2 weeks, no later than POC due date of 11/9/22. Based on observation and interview the blue area rug was stained and particles were visible during the morning inspection conducted by LPA. This poses a potential health, safety, or personal rights risk to children in care.

101170(e)(2) · 101170(e)(2)-Criminal Record Clearance: All individuals subject to a criminal record review...shall prior to working, residing or volunteering in a licensed facility: Request a transfer of a criminal record clearance as specified in Section 101170(f).

Director, Andrea, stated she is attempting to register for a guardian account to transfer/ associate staff herself. She will ensure staff is associated by POC due date by either completing Guardian registration or submitting Transfer Clearance by fax to regional office. Based on record review and interview 3 staff were not associated to the facility prior to working. This poses a potential health and safety risk to children in care. Request Denied

101223(a)(2) · 101223(a)(2) Personal Rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs.

Director, Andrea, will inform parent sleep sack can not be used in child care without a medical exemption. Center will cease use or have parent bring in medical exemption. Based on record review and observation the child was swaddled in a sleeper provided by the parent without a medical exemption on file. This poses a potential helath and safety risk to the child.
