Based on the information obtained, there were multiple minor incidents involving Child #1 (C1). As a result of these repeated, unwelcome interactions, children began exhibiting signs of emotional distress. This pattern of behavior presents a potential risk to the health, safety, and personal rights of children in care.

Based on the information obtained, facility staff failed to report multiple minor incidents to parents. This lack of communication poses a potential risk to the health, safety, and personal rights of children in care.

Based on record and file review, staff S1 (employed in 8/4/2025) did not have fingerprint clearance on file (Guardian database). Based on the information obtained, between October 2024 to present, there were more numerious separate incidents wherein children were bit, scratched, grabbed or had his/her hair pulled by another day care child this poses an immediate health, safety or personal rights risk to children in care.

101212(d)(1)(B) · Upon the occurrence, during the operation of the child care center of any of the events specified…a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report… shall be submitted to the Department within seven days following the occurrence of such event.

Licensee will conduct a staff meeting regarding reporting requirements by 11/3/23 and submit attendance sheet to the department. Based on file review and interviews, the licensee did not report an incident within 7 days in which medical atention was sought. This posed a potential risk to the health and safety of children in care.

Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in that there was a retractable box cutter in a drawer in Stems B that was accessible to children which poses/posed a potential health, safety or personal rights risk to persons in care.
