The director states that she conducted training on transitions and supervision with all current staff. The Director provided a copy of the training agenda and staff sign in sheet for the staff training held on 10/11/21. The director will submit a signed and dated statement explaining how she will ensure that staff are following the correct procedures for transitions/supervision. The director will also provide verification that management is monitoring how staff are implementing supervision/transition procedures to LPA via email by POC due date. Based on interviews conducted by LPA, a child (C1) was left unattended on the playground for approximately 10 minutes on 9/20/21. This poses a potential health and safety risk to children in care.
