The room’s cubby shelves were removed for greater room visibility. Staff said teachers will be retrained on face to head counts on 02/16/2024. Staff said they will provide LPA with a copy of the staff training agenda and staff training sign in sheet no later than 02/23/2024. Based on observations and interviews, the Licensee did not comply with the section cited above in that a child was left without the visual supervision of a teacher, which poses as an immediate health, safety or personal rights risk to persons in care. Staff completed in-service re- training about supervision on 10/28/2022; director provided LPA with written documentation of this in-service training and attendance documentation log. Based on observations and interviews, the Licensee did not comply with the section cited above in that a child was left without the visual supervision of a teacher, which poses as an immediate risk to children in care. Director said she will provide the Department with a written statement describing what steps staff will take to ensure this incident does not recur. an unauthorized person retrieved a child from The director has individually informed and written to all staff of the change in authorized individuals who may retrieve the child.
