Assistant Director Lynn Thedell states meetings with staff have been conducted regarding supervision. The last meeting was on 4/11/25. Attendance sheet was provided to LPA. Based on observations, interviews and record review, the facility did not ensure children in care were not bitten by another child in care, which posed a potential Health, Safety or Personal Rights risks to persons in care. A specialist was consulted with for recommendations. Sensory items and other types of toys have been introducted into the classrooms. Resources such as books have been provided to staff.

Director states that she during the chaos of Covid, she forgot to follow up on the clearance process and was just recently reminded of it. She immediately placed Staff #1 on leave pending the completion of the clearance. She will ensure that the clearance is complete prior to Staff #1's return. Based on interview with Director, Staff #1 was hired and allowed to work before the background clearance was completed. This is an immediate risk to the health and safety of children in care. She will ensure all future hires have a completed clearance prior to hire. SUPERVISOR'S NAME: Renesha Askew

Director states that she was made aware of the incident on the second occasion which occurred approximately one week ago. The prior was approximately two weeks ago. Immediately upon learning of the incident, she spoke with all staff about proper supervision on the changing table and posted the rules at Based on interview with Director and Staff #2, Staff #1 stepped away from a preschool child on the changing table, for approximately 10 seconds, on twodifferent occasions. This is a potential hazard to the health and safety of children in care. every changing table in the faciltiy. A copy was provided for LPA at this visit.
