Deficient Practice Statement Based on observation and interviews, the facility did not comply with the section cited above in that they were not aware of the requiement to have form LIC 9227 in place for children under 12 months. Infants were observed sleeping on their backs and staff stated that they turn them back if they flip over, therefore the missing form poses a potential rather than immediate risk to infants in care.
