as observed by The director Jackie Corey stated the facility immidiately implemented corrective measures, including retraining staff on child release procedures, conducting name to face headcounts during transitions, and requireing staff to verify identity of authorized representatives and manually verify child sign-in/out records prior to relaeasing chilren from care. The facility reported to the Department that on 6/9/26 child (C) was released through the facility gate without staff supervision and remained unsupervised for approximatley 2 minutes posing an immidiate risk to health and safety of child in care. The director Corey stated parents were notified of facility release procedures and structed to enter the facility for child pick-up and sign children out prior to departure. Director Corey also issued a written disciplinary warning to Staff (S) and provided proof of corrective actions to LPA on 6/18/26.
