Staff meeting held February 4th regarding supervision of chidlren in care. Based on interviews the facility did not ensure the supervision of a child during restroom use. The director states that S1 has been suspended for five days with no pay and placed on a performance plan. Based on interviews, record review and video footage, on 3/23/23 at approximately 2:50 p.m. a staff member (S1) slightly pulled a child (C1) by the hand at a pace C1 had difficulty keeping up with. The poses a potential health, safety or personal rights risk to children in care. Lead Teacher Diana Padilla states that she will have the director conduct a staff meeting on childen's personal rights and she will send LPA a copy of the meeting agenda and staff sign in she via email by 5/5/23.
