Director stated they will email LPA a sign in/out sheet for staff that attended the training and watched the CDSS supervision video by the due date. Based on interviews, the licensee did not comply with the section cited above in that, Child 1 (C1) was on the playground alone without teacher supervision, which poses an immediate health, safety or personal rights risk to persons in care. evidenced by Director states she will submit a detailed plan to ensure all children are being supervised at all time including transitioning from the classroom to the restroom and vice versa, to LPA by due date. Based on LPA's interviews conducted with 4 staff, all interviewd staff confirmed that C1 was left alone in the locked restroom for approximately 2-10 minutes, which posed an immediate risk to the health, safety and personal rights of the children in care.
