Facility must adjust schedule of staff/parents to ensure that no staff are left operating over staff to child ratio.Facility will submit an updated LIC 500, Personnel Report. Facility must also submit proof of the schedule change of staff that has happened since the beginning of the year. Based on observation and record review, the licensee did not comply with the section cited above. During the course of the investigation, it was determined by interviews, observations, and documentation that staff have been operating over staff to child ratio which poses an immediate health, safety or personal rights risk to persons in care. Facility will implement appropriate procedures to ensure staff are not operating over staff to child ratio. Written procedures should include what steps staff should take when they are about to meet 1 to 12 ratio and when teachers are absent/running late. A meeting must be conducted to ensure that all staff are made aware of the policies. Staff acknowledgement of meeting must be submitted to LPA after it is conducted. Facility will submit written procedures and updated Personnel Report to LPA Tapia-Mandujano by 12/12/25.

Facility has put in a maintenance request to install privacy slats in the fence to prevent children from climbing the fence. Facility will submit the supervision policy and how they plan to prevent these incidents from recurring. Based on interviews and record review, a child (C1) was able to climb over the playground fence leaving the premise without the direct supervision of staff. This poses an immediate health, safety and personal rights risk to children in care. Facility will have a training about Supervision policy and practices to ensure that all staff are aware of the supervision policy. Facility will submit to LPA Tapia-Mandujano both the written plan and the training notes with signatures obtained of all staff present by July 18th, 2025.

Facility administrators acknowledge that there cannot be more than 91 preschool age children present at any give time. Facility will submit a written plan on how they will resolve this by the end of the day of June 18th, 2025. Based on observation, interview, and record review the facility did not comply with the section cited above as the facility has 94 preschool age children present with 124 enrolled children exceeding the licensed capacity of 91 preschool age children. This poses an immediate health, safety and personal rights risk to children in care. If facility decides to increase capacity, increase of capacity application will be submitted to the department. Site Supervisor stated there are usually three staff in the classroom. SCSD-Program Director stayed in the classroom to be in teacher to child ratio. Both SCSD-Program Director and Site Supervisor are aware of teacher to child ratio. Based on observation and record review, the licensee did not comply with the section cited above. LPA observed two fully qualified teachers with 25 preschool children in Room 24 (Dragonflies), which poses a potential health, safety or personal rights risk to persons in care. Facility will submit a written schedule for each classroom with number of enrolled children. Facility will also develop a written plan on how they will ensure that there is always coverage in the event staff are not present. Plan should include meaures for staff who call in last minute and are scheduled to be off. Written plan will be submitted to LPA Tapia-Mandujano by 6/20/25.
