Licensee stated a qualified teacher had called out sick on this date. Per licensee, fully qualified staff are hired and work at the facility. Licensee also stated a fully qualified staff was present in the Honeybees classroom, however, moved classrooms during LPAs' visit. Based on observation, interview and record review, 18 preschool children were present in the Honeybees classrooms with 2 staff (including TA1). LPAs observed TA1 does not have Early Childhood Education units, therefore teacher to child ratio was not met. This poses a potential health, safety or personal rights risk to children in care. Staff qualifications handout was provided during today's visit. Licensee stated TA1 will either register for Early Childhood Education units or be moved to a classroom with fully qualified staff to ensure ratio is met. Deficiency Dismissed

Facility self reported unusual incident to the department. An emergency staff meeting was conducted January 12, 2026, discussing children’s personal rights and how to appropriately work with children. Staff member who inappropriately handled a child received a write up from director and was terminated from facility. Based on interviews and record reviews, a staff member was observed to grab a child, pick the child up and force the child to sit down in a rough, inappropriate manner. This poses an immediate health, safety or personal rights risk to children in care. Civil penalty was assessed. Staff will have an additional training through the department on how to appropriately handle children. LPAs will refer director to child advocacy for personal rights training.

by evedice by Facility will have a staff meeting for all staff to be aware that toddlers cannot exceed a maximum group size of 12 and discuss new procedure when using the restroom for children. Facility has a daily routine schedule posted in each classroom with times when their classroom used the restroom and will also post a restroom schedule in each classroom for all staff to be made aware if the restroom is available. Based on observation during physical plant inspection, LPA observed there were 13 toddlers with 3 staff in the bathroom area exceeding the maximum group size of 12, which poses a potential health, safety or personal rights risk to persons in care. Staff will also use their walkie talkies to communicate when they are going to the restroom to ensure that restroom is available. Facility will submit proof of the staff meeting agenda with the procedure to follow and send pictures of the schedule posted in each classroom to LPA Tapia-Mandujano by September 2nd, 2025. Facility will discuss the incidents that were observed with the appropriate staff to be made aware of incidents occured. Facility will submit a written plan on alternate ways to handle the incidents observed to not violate the personal rights of children. Based on record review of video footage there were three separate incidents observed of a child being handled in a rough manner. This poses an immediate health, safety and personal rights risk to children in care. Facility will train all staff on the alternate ways to handle children to not violate the children's personal rights. Written plan and training notes must be submitted to LPA Tapia-Mandujano via email by August 21st, 2025.

LPAs found that S1 did have an active fingerprint clearance during the visit. Director filled out LIC9182 and provided it to LPAs during the visit. S1 was then associated to the facility. This deficiency shall be cited and cleared the same day of the visit. Based on observations, record review, and interviews, LPAs found that a staff member, S1, did not have association to the facility during the visit.
