101223(a)(3) · (a) The licensee shall ensure that each child is accorded the following personal rights: (3)To be free from corporal or unusual punishment… or other actions of a punitive nature… This was not met as evidenced by Site Supervisor stated the facility is in the process of offering additional coaching/training for staff regarding managing children with challenging behaviors. The facility will be responsible for providing LPA with documentation regarding training Based on LPA observation and interview, Staff admitted to enclosing a child in care in an arm circle, limiting their movement for a brief period of time. This poses a potential health/safety or personal rights risk to children in care. and coaching topics related to personal rights, and/or managing children with challenging behaviors. Training materials in addition to signed acknowledgement from staff will need to be submitted to LPA on or before the POC date of 03/13/2026. 101238(a)(1) · (a)The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This was not met as evidenced by Licensee agrees to (1) establish a plan of action on how the facility maintain a rodent free environment. (2) Licensee agrees to have the facility professionally inspected and treated for rodents and (3) agrees to submit a copy of 1 and 2 to CCLD by POC due date of 10/15/2025.
Based on observation, the facility is not free from rodents. LPA observed rodent droppings and glue traps at the facility during the week of 08/18/2025 within areas inaccessible to children. Pertinent parties stated rodents were observed at the facility during the entire month of August 2025 Records revealed the pest control vendor is not treating the facility for rodents but provides general pest services. This poses a potential health, safety or personal rights risk to persons in care.
