Based on interviews and facility records the Licensee did not ensure construction Per Administrator the fence panel was removed on 4/3/24. The Center has and will discuss the importance of keeping a safe environment with all outside companies who complete work on the property. Individual staff training regarding the equipment was inaccessible on April 3, 2024, when a metal fence panel was in the preschool outdoor activity space. The fencing was not made inaccessible and fell on C1 causing injuries to the head and arm. This is an immediate health and safety risk to the persons procedures for outdoor inspections will be completed by 4.12.24. A checklist was created for the staff to utilize during the safety checks. A copy of the completed checks will be submitted to the Department everyday for the next two weeks starting 4.15.24.
