Facility will submit proof (pictures) of cleaned areas identified with debris to the department by POC due date 06/10/24 or earlier. Based on LPA observation the licensee did not comply with the section cited above in that LPA observed the following areas with debris: toilets, sink, gates, carpets, tables, chairs, and floors which poses/posed a potential health, safety or personal rights risk to persons in care.

101439(b) · Infant Care Center Sleeping Equipment 101439(b) - A crib or portable-crib meeting United States Consumer Product Safety Commission safety standards shall be provided for each infant who is unable to climb out of a crib.

Based on observation the licensee did not comply with the section cited above in that at 1:34PM, LPA observed a non-walking/crawling infant sleeping on a pillow round on the carpet which poses/posed an immediate health, safety or personal rights risk to persons in care.
