LPA observed two (2) aids (S1 & S2) supervising six (6) infants. This poses an immediate risk to health and safety of the children in care. AB633 Parent Notification is required. According to AB 633, all parents of children currently enrolled and any future children being enrolled for the next 12 months must be provided with this report which contains this type A deficiency.

By 04/30/2024, Licensee/Director stated that they will transfer S-1's fingerprints. S-1's fingerprints were transferred during today's inspection. Based on observation, interview, and record review, staff (S-1) who was present at the facility has cleared fingeprints, but fingerprints are not associated to facility. LPA observed rip mattresses in all infant cribs, two broken cribs, broken blinds, unsanitary carpet in crib area and in infant indoor playground structure, stains in walls & chip wall paint.
