Based on tour observation indoor and outdoor of the toddler & preschool, LPA toured outdoor play areas and observed children's bikes, cars, waffle blocks, play house, chairs, easel, and broken toys, one play structure was rusted in several places, a part of the play ground structure was removed leaving behide a piece of concrete with screws sticking out, and uneven walking surfaces tripping hazard to children in care (toddler & preschool outdoor area). LPA observed disrepair items were not placed inaccessible to children in care. LPA toured the preschool classroom and observed broken blinds, dusty ceilings & indoor play area stairs dusty, sprider webs, rip carpet, dirty rug, stain walls & chip paint, a lose baseboard in children bathroom.
