LPAs observations Director will meet with Licensee and board of director regarding the deficency. LPAs oberved the following throughout the facility; several parts of the vinyl on the floor lifting & exposing the carpet underneath it, exit door with a chipped paint, missing small part of the base board exposing chipped paint, small exposed spot of the carpet, carpet was not clean showing brown to gray stains, the stair there was a missing the vinyl exposing the wood underneath it, and insufficent cushioning of barks undert the play structure.
