Facility will have staff review trainings for supervision in a Child Care Center and will develop new procedures for census counts during fire alarms/drills to ensure all children are appropriately accounted for, and will email LPA a copy of those trainings by 22 November 2025. Based on observations, interviews, and record review, the licensee did not comply with the section cited above by having a self-reported incident on 18 November 2025 where a child was left without supervision in a classroom for approximately 10 minutes during a fire alarm, which poses an immediate health, safety or personal rights risk to persons in care. Facility has been assessed a POC cleared letter as of 02/10/2023.
