101223(a)(3) · 101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning.
Director stated that S1 was removed from the facility while admin was conducting an investigation. S1 was also transfered to a new SETA facility. Based on the Directors, staff, parents, and a child's interviews, it was found that S1 was handling children in a rough manner and yelling at them while in care. This posed an immediate health, safety or personal rights risk to persons in care.

The Director have conducted a Supervision Training with the staff at the end of January 2026. Based on the Director and staff interviews, it was found that a child sustain injury while in care, but staff is not aware of how and when did the injury occurred. which poses an immediate health, safety or personal rights risk to persons in care.

Director stated that the water fountatin has since been capped off, shutting off the water drinking fountain in the toddler classroom. Director showed LPA that the water fountain does not work.
