101229(a)(1) · (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.

The facility investigated the incident on 4/27/2026. Director stated that S3 is no longer employed at the facility. Director also put several carabiners to secure the latches, spoke to all the staff to ensure the gates are completely locked, and conduct proper supervision. Based on interviews and the submitted a LIC624 UIR form, P1 and S3 found three children in the toddler yard alone with a lack of supervion. This poses an immediate health, safety or personal rights risk to persons in care.
