This requirement has not been met by evidence: Facility failed to submit a report to the Regional Office. This is considered as a potential risk to the children in care. Director stated that they did not know what to do regarding reporting because they have not had an incidents in the facility for a long time. Director submitted the UIR during inspection. LPA cleared the deficiency. 101229(a)(1) · No child(ren) shall be left without the supervision of a teacher at any time... This requirement has not been met by evidence: Staff#1 left a child unattended in the playground. This is considered as an immediate risk to the children in care. *THIS IS AN AMENDED DOCUMENT Director stated that they have suspended the staff#1 for the remainder of the week and trained the staff involved in the incident. The facility was issued a civil penalty of $500. LPA cleared the deficiency.
