Based on interviews conducted with S2 and S3, it was disclosed that S1 The director has already held a staff meeting reviewing children's personal rights on 03/28/2025. Director has provided LPA with a copy of the meeting agenda and signatures of staff who attended. Per director, S1 was terminated as a result of the incident that occurred. purposely taped C1's shoes to prevent C1 from continuing to take off their shoes. This poses a potential health and safety risk to children in care. 101430(a)(3)(A)(4)(a) · Infant Care Activities (A) Staff shall place infants up to 12-months of age on their backs for sleeping. (4) Infants with an Individual Infant Sleeping Plan [LIC 9227 (3/20)] that have Section C of the form completed and signed by an authorized representative shall be placed on their back when first laid down to sleep. In the event the infant changes position, the infant may remain in the alternative position. (a) Upon staff observation that the infant is capable of rolling from their back to their stomach and stomach to their back, the provider shall fill out Section D of the Individual Infant Sleeping Plan [LIC 9227 (3/20)], notify the authorized representative, and obtain the authorized representative's signature on the individual sleeping plan no later than the next business day following observation.
Deficient Practice Statement Based on observations , interview and records review , the licensee did not comply with the section cited above. During the file review LPA did not observe sleep log in the infant file.Facility Represantative was unable to provide sleeping log for any infants enrolled under 12 months , which poses/posed a potential health, safety or personal rights risk to persons in care. Per DIrector, staff meeting regarding training to approaching biters and development, also Workforce Registry. Copy of email regarding Bite Policy to parents, and copy of Agenda that you will be discussing with Staff at meeting. To be submitted to LPA by POC date Based on interview, Licensee did not ensure a child #1 was provided safe accommodation from another child with known instances of biting, which resulted in five bites on child #1's arm. This posed a immediate risk to the health, safety, and personal rights of children in care.
