LPA brought the worn changing pad to the directors attention and the changing pad was changed immediatly during LPAs visit on 6/3/25. Based on observation, Facility did not comply with the section cited above, as LPA observed a worn changing pad, which poses a potential health, safety, or personal rights risk to persons in care.

Based on observation LPAs observed 17 infants in a classroom with 4 staff, which poses an immediate health, safety, or personal rights risk to pesons in care Director moved an additional staff member into the classroom. Deficiency cleared during inspection.

as when the parent picked up the child from the facility, the child's clothing had a poop stain on it in the back and the child's neck area was very wet. Pictures shows either the child's diaper was not changed causing the poop stain or the child was not changed into clean clothing after having poop diaper changed and child not cleaned up around the neck area prior to being picked up by parent, This is an immediate risk to a child. Owner, Bonnie Lee, was informed if deficiency is not corrected by due date a civil penalty may be assessed.

as an infant was accidentally given the wrong bottle. The child was crawling and grab a bottle that sitting next to a staff. This is an potential risk to a child the names on the bottles before the infant are given a bottle. Owner talks about all center challenges at the bi-weekly zoom meetings. Owner submitted a written statement and sent to CCL Deficiency cleared at time of inspection.

Owner stated she will remind staff at all staff meeting that pillows shall not be used in the crib. The large size pillow was removed from infant baby breathe classroom at time of inspection. as evidence by based on photos an infant was sleeping on it's back on a regular size bed pillow. This is an immediate risk to a child. Deficiency Cleared. LIC 9224 was given and discussed with owner at time of inspection.

based on photos an infant was playing with a cloth toy and had it in its mouth and then given to another infant to play with within minutes before being sanitized. This is an immediate risk to a child Buckets are kept on the shelf in the infant Baby's breath classroom. LIC 9224 was given and discussed with owner at time of inspection. Request Denied

based on photos the facility does have a baby lounger that was being used and has been recalled. This is an immediate risk to a child LIC 9224 was given and discussed at time of inspection.

Deficient Practice Statement Based on record review a teacher only had three units in the infant cherry blossom classroom. She was being used as a qualified teacher and she did not have the required 12 units. Staff number two in the infant baby's breath classroom only had 9 units of the 12 units needed, she was also being used as a qualified teacher. This is a potential risk to children.
