as A work order to repair the windows was submitted on 1/21/2026. Maintenance and a window vendor observed repairs needed at the facility on 1/28/2026, and new windows were ordered. All staff were told not to open windows, and windows were locked. Staff were required to complete a training on evidenced by staff interviews and an Unusual Incident/Injury that was reported on 1/16/2026. On 1/15/2026, a window closed on the finger of Child 1, after Staff 1 opened the window, although there were signs on the window not to open it. Some staff were aware the the window needed repair for approximately 3 years, however a repair was not ordered until after the incident. supervision and safety. Director will submit meeting notes with staff signatures of the training that was completed to LPA by the POC due date.

Director stated that new windows were ordered on 1/28/2026, and that the current windows are locked where no staff can open them. A maintenance request log was created for staff to list repairs needed in their room, which the closing manager must that staff knew windows at the facility were not functioning properly for approximately 3 years. This poses/posed a potential threat to the health and safety of children in care. review and submit a repair order for. LPA will return by 3/02/2026 to observe repairs.

Facility self-reported that All families were notified of the incident as well as the district leader. Facility has began the process of replacing the iron rod fence with a privacy fence that is not climbable. Facility will also add additional fencing to the chain link fence that will also make it unclimbable. Director stated that the new fencing should be completed as soon as possible and a work on May 10, 2022, Child #1 was absent from the facility for approximatley 15 minutes, and found by the fire department at the playground of a neighboring facility. This is an immediate risk to the health and safty of children in care. order has been placed.
