Staff 1 and Staff 2 are no longer employed at the facility. Director will host a meeting with current staff that reinforces proper techniques for redirection and discipline. Meeting minutes will documented and proof of completion and staff attendance shall be sent to LPA by 07/12/2024. as evidenced by staff interviews and file review, which corroborated the allegations of former staff rough handling children, speaking to children inappropriately, and threatening and using unsual or corporate punishment. This posed an immediate risk to health and safety of children in care.

staff interviews and incident The staff followed the emergency medical procedures during the incidents. The staff have developed a plan to ensure that the environment does not pose a risk for child #1 to come into contact with the allergen by designating a staff to supervise child #1 during and after feeding times. Staff are report submitted on 04/18/2023. It was determined that child #1 was not afforded a safe healthful accomodations. On 04/03/23 and 04/17/2023, child #1 suffered allergic reactions after coming into contact with an allergen during lunch. This posed an immediate risk to the health and safety of the child in care. also working on other measures to lower the risk of exposure to the known allergen. SUPERVISOR'S NAME: Keven Peters

observation, interviews, and file review. Staff 1, 2, 3 were observed supervising infants alone without the required qualifications. (It was concluded that Staff 4 also supervised infants alone without infant teacher qualifications.) This is an immediate risk to the health and safety of children in care

file review. LPA observed that Staff 1, 2, 3 and 4 did not any of the required qualifications, but were supervising infants wihtout the prescence of a fully qualified teacher. This is a potential risk to health and safety of children in care job posting/recruitment package as verification of the hiring process. Director will reach out to other Kinder Care Centers to see if they have any qualified infant teachers that can substitute temporarily.
