The staff noticed the child was given the wrong bottle and stopped feeding child immediately, corrected action and notified management. Facility self reported incident to licensing. Parent was notified and staff training was given. Deficiency cleared.

101216.1(b) · Teacher Qualifications and Duties Prior to employment, a teacher shall meet the requirements of (b)(1) or (b)(2) below

by Director stated the staff will not be left alone with children. Other staff did not know the staff was not qualified. The staff is currently attending school to become qualified. Deficiency cleared. An aide was left alone in a classroom with preschool children. This is an immediate risk to children. LPA Owens gave and disccused the LIC 9224 with Director at time of inspection.
