the facility reporting via Unusual Incident Report Staff 1 was given a final warning. Director gave training to staff on food allergens and safety. A training agenda with staff signatures shall be sent to LPA by 08/15/2025. that on 08/01/2025, C1 was given dairy by Staff 1 during snack time. Facility has documentation via Health History and the Infant Needs and Services plan that the child shall not have dairy. This posed a potentital threat to the health and safety of the child in care.
