Facility self-reported that on 01/16/2026 Child 1 and Child 2 eloped from the facility unsupervised. The children were found at the residence of Child 1 approximately 30 minutes after the facility was aware the children were not present. This posed an immediate risk to the health and safety of children in care. Doors from the bathroom that open to the street will now be locked.
