as evidenced Staff 1, 2, and 3 (who were supervising children when this happened) were let go. Director held a staff meeting with current staff regarding new protocols for supervision of children. There will be a designated staff to supervise the bathroom area. Staff will conduct census checks every 15 minutes. by an Unusual Incident Report received on 01/20/202. Facility self-reported that on 01/16/2026 Child 1 and Child 2 eloped from the facility unsupervised. The children were found at the residence of Child 1 approximately 30 minutes after the facility was aware the children were not present. This posed an immediate risk to the health and safety of children in care. Doors from the bathroom that open to the street will now be locked. This requirement is not met as evidenced by: results from a lead test of a faucet/fountain on 12/09/22 that indicated an exceedance of 5.5 ppb. which posed a potential health, safety or personal rights risk to persons in care.
