Facility has implemented a requirement for staff to do a double roll call in the morning, and for staff to stay an additional 15-20 minutes after children have been dismissed to ensure that no child has been left alone. by an Unusual Incident Report received on 10/28/2025. Facility self-reported that on 10/27/2025, Child 1 was left alone in the room from approximately 7:40am-9:3ham. This posed an immediate risk to the health, safety, and personal rights of children in care. Site Coordinator stated they have implemented having a staff at the children's bathroom and double checking the count of children going to restroom by way of walkie talkie and returning from restroom. LPA observed and witnessed the new procedure being implemented at time of inspection.
