Staff 1 was given a final notice and any further discipline regarding violation of children's rights will result in a termination. Staff 1 is no longer working in the same classroom as Child 1. Director will conduct personal rights a self-reported Unsual Incident Report from the facility. On 2/13/2025, Staff 1 forced Child 1's head down on their cot with one hand during nap time. 101216.3(a) · Teacher-Child Ratio. (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance.
LPAs observations. LPAs observed 13 children supervised by one staff in three separate classrooms. This occurred before Staff 2 arrived to Director stated that she will create a new schedule to have staff come in earlier to help with the time window of 8:00-8:30 am. Director stated that the food will be prepped the night before so no one has to help prepare/serve food in the morning. Director stated that a staff member who called out will assist a room in ratio. During the initial census LPA observed Staff 1 supervising 12 children before she was replaced in ratio by the director. Director stated that Staff 1 is an aide and does not have qualifications for a teacher. This is considered an immediate risk to the health and safety of children in care. return to work tomorrow. Director stated that she is in the process of hiring two additional staff for the facility as well. Director stated that she and assistant director will be able to step in as needed in the classrooms. LPA will conduct an inspection to observe ratios.
