LPA discussed with Director that cribs must be free from all loose articles and objects, including blankets. Staff members immediately removed the blankets during the visit. Based on observations, the director did not comply with the section cited above as LPA observed three infants sleeping with blankets in a crib which poses an immediate safety risk to children in care. LPA reminded Director to maintain required documents for all staff. Director agreed to provide LPA with completed LIC 503 forms and proof of negative TB test results for S1, S2, and S5 by 08/08/2025. Based on observations and records reviewed, the director did not comply with the section cited above as director did not ensure a health screening, including a negative TB test, was completed for three out of five staff present which poses a potential health risk to children in care. 101428(d)(1) · 101428 Infant Care Personal Services (d) When changing an infant's diapers, the following shall apply: (1) Each infant shall be diapered on a changing table.
LPA discussed diapering requirements with Site Supervisor. Site Supervisor agreed to conduct diapering training for staff and provide LPA with a copy along with signatures from staff for verification by 09/13/2024. A civil penalty of $250 is being assessed and provided. Based on observations, interviews conducted, and records reviewed, it was revealed that a wooden bench is used to change infants standing up and changing pads are not utilized as an alternative to changing tables which poses a potential personal rights risk to children in care.

Facility agrees to start utilizing the changing table in the toddler classroom. Facility plans on purchasing stairs to assist staff with guiding the infant/toddlers onto the changing tables and submit photos to CCL by 5/17/24.
Based on observations and interviews, staff are not utilizing the changing table in the toddler classroom (18-30 Months), which poses a potential health, safety or personal rights risk to persons in care
