Facility will cease use of sleep sacks unless an exception has been requested and granted by the department. Based on LPA direct observation during tour at 11:15AM, 4 infants were observed in sleep sacks and information received is parents or facility provide sleep sacks without exceptions in place. This poses a potential health, safety or personal rights risk to infants in care.
