Supervision shall include visual observation. Civil penalty of $500 assesed. Staff was aware children were missing, however, did not follow protocols per staff interviews. Based on LPA interviews, staff did not provide care and supervision for two children who where left unattened at the facility playground. This is an immediate health and safety risk to the children in care. Site Director, will conduct a safety training and name to face protocol during transition from class to playground.
