Facility will submit a written plan of action for ensuring children are accorded dignity in their relationships with staff including how facility will monitor concerns with employees, to the department by POC due date 07/07/23. Based on interviews conducted and records reviewed, facility did not accord the dignity of children in care in their personal relationships with staff. Deficient Practice Statement Based on [(observation) the licensee did not comply with the section cited above in that LPA observed 1) Outdoor planters with exposed rotting/splintering wood 2) Flooring torn and soft and lifting away from wall by toilets 3) Staff store room with missing and disintegrating ceililng tiles 4) Tray under water access for toilet- appears for plumbing or possible ceiling leak which poses/posed a potential health, safety or personal rights risk to persons in care.
