Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in at 9:26 AM during today's inspection 4 of 6 play yards were observed to have loose fitted and dislodged sheets on the mattresses designated for infant children, which poses a potential health, safety or personal rights risk to persons in care. Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above in at 9:26 AM Bedroom 3 designated as a napping room was observed 5 play yards, 1 play yard filled with miscellaneous items, 4 play yards observed to have loose sheets, blankets, pillows and plush stuffed toys. At 9:45 AM LPA S.Brewer observed a 6th play yard located in the designated indoor living room/daycare area to have blankets, pillows and plush toys which poses a potential health, safety or personal rights risk to persons in care.
