Based on LPA record review & interviews, it was determined a daycare child submitted verbal & written statements to the licensee that a staff yell at daycare children, causing children to cry & creating an uncomfortable environment, which posed an immediate risk to the health & safety of children in care. Per director, they agree to submit the plan and training curriculum by 02/02/2024. 101170(e)(2) · Criminal Record Clearance. (e) All individuals subject to a criminal record review...shall prior to working, residing or volunteering in a licensed facility: (2) Request a transfer of a criminal record clearance as specified in Section 101170(f).
During this inspection, Director stated Staff #1 is present at the facility two to three times a week and stays at the facility for approximately 45 minutes. Director completed the LIC9182-Criminal Record Transfer Request and provided a copy to LPA Hogue during this inspection. Director also provided a copy of Staff #1 picture ID, along with a copy of DOJ applicant Based on LPA's review of facility associations, LPA observed an outside vendor instructor present and alone with children in Classroom #3. Staff #1 has a criminal record clearance, however the Licensee/Director did not request a criminal record transfer request prior to Staff #1 working or being present at the facility. This poses an immediate health, safety, and personal rights risk to children in care. fingerprint response, and LIC508-Criminal Record Statement during this inspection.
