Based on interviews conducted, a staff member aggressively moved a child’s hand from a shelf and using a harsh tone of voice to redirect the child, which is a potential risk to health and safety and personal rights of children in care. Facility will implement corrective action pursuant to section CCR 101704 for immediate cessation of outlets testing with action level exceedance until it is replaced and retested pursuant to section CCR 101705 and returns a result at or below the Acton level. Facility will notify CDSS with completion within 30 days. Based on records review of required lead testing, the facility had lead values of 5.5 or above on water outlets: Outlet B (33 ppb), and Outlet E (14 ppb). This requirement has not been met as evidenced by Early Education Administrator understands the importance of this regulation and meeting the Facility’s reporting requirements. Facility will inform all parents/guardians of their entire preschool program of confirmed cases of a communicable illness as well as report incidents of two or more cases to licensing.
Based on interviews and records reviewed LPA discovered the facility had knowledge of confirmed cases of hand foot and mouth and only informed parents of one of three classrooms. Additionally Licensing was not informed. This poses a potential risk to the health and safety of children in care Facility Administrator will submit to licensing on or before the stated POC date of 11/23/2022 a statement of understanding of this regulation and written plan to meet their required reporting requirements.
