T1 and T2 were put on administrative leave during internal investigation and eventually separated from facility. A supervision retraining will be conducted during staff meeting on July 24th, 2025. Management will be conducting observations during transition times for all classrooms. Based on interview and record review, the facility did not comply with the section cited above as facility self reported that there was a child left alone in classroom located on second floor during transition to outside time, which poses a potential health, safety or personal rights risk to persons in care. Director will submit the retraining agenda conducted with staff regarding supervision and transitions procedures and policies. Director will continue to check in on staff during trasitions to ensure the policies are being followed.
