Facility conducted an investigation plan and took immediate action based on internal policies and procedures. Facility will reiterate the Supervision policies for indoors and outdoors with all staff during a staff meeting. Based on interview and record review, the facility did not comply with the section cited above as facility self reported that there was a child left alone inside the classroom during outdoor play, which poses a potential health, safety or personal rights risk to persons in care. Facility will submit a staff meeting agenda, policies that were reviewed. Facility will also obtain a signature from all that that they acknowledge the policies that were reviewed. Documents will be submitted to LPA Tapia-Mandujano via email no later than 5/16/25.
