Based on observation, Interview, and record review, On 4/22/24, LPA Tapia-Mandujano observed and was told that S1, S4, and S5 have been left alone with children without the direct supervision of a fully qualified teacher, which poses a potential health, safety or personal rights risk to persons in care. Facility will submit submit an Action Plan to ensure that no unqualified staff is being left alone without the direct supervision of a fully qualified teacher.
