Facility agrees to train all infant staff members on Children's Personal Rights and meeting the needs of the children in care. Facility will submit a statement and training agenda to LPA via email by POC due date. Based on evidence obtained, interviews & S6 admission to covering the child’s mouth to prevent child from crying and physically restraining children on two different occasions, which poses/posed an immediate health, safety or personal rights risk to persons in care. Director made sure to removed the object attached to the side of the crib during today's visit when informed by LPA Lovest. Based on LPA Lovest observed S1 place C6 on the crib where a portable sound machine was attached to the side of the crib, which poses/posed a potential health, safety or personal rights risk to persons in care.

The facility will make sure the broken toy container is disposed and replaced by POC due date. A written statement of completion will be submitted by POC due date. Based on LPA observation the plastic toy container holding the soft blocks is broken, not maintained in good repair and safe condition, which poses/posed an immediate health, safety or personal rights risk to persons in care.

Facility agrees to washed and disinfected at least daily, or more often if necessary before allowing the infants to play with the soft blocks. A written cleaning plan will be submitted to LPA via email by POC due date. Based on LPA observation and inspection the infant toys located outside in the designated infant outdoor play area are not clean and have sticky residue, which poses/posed an immediate health, safety or personal rights risk to persons in care.

Facility ensures staff will wash hands before feeding and after diaper chaning in the infant classroom. Facility agrees to submit a written diaper changing and feeding procedure signed by infant staff to LPA via email by POC due date. Based on observation the facility did not comply with the section cited above,LPAs observed 4 infants diapers were changed by S5 and 1 infants' diaper changed by S3. S5 and S3 did not wash hands after changing infants' diapers. LPAs observed 4 infants fed by S1 and 2 infants fed by S8. Based on observation of S1 and S8, did not wash their hands before the feeding which poses/posed a potential health, safety or personal rights risk to persons in care.
