Facility has went over Supervision expectations with each teacher/classroom. Facility has made substantial changes to secure possible exits for children. Head of School agrees to conduct a formal staff training with regards to Supervision. Based on interviews conducted, documentation reviewed, and observations made, it was revealed that the facility failed to provide adequate supervision for the two subject children, due to them being to leave the facility. Head of school agrees to submit documentation of the trainings, including the agenda and staff sign-in sheets once completed. Head of school also agrees to submit a plan of action moving forward to prevent this type of incident from happening again.
