The facility has already implemented corrective measures, including adding a signature line on medicine records for double verification, retraining management staff on medication procedures, and requiring all medications to be administered in the front office by management staff. Based on interviews and record review, it was determined that staff administered the wrong medication to a child in care, resulting in the child being taken to the doctor as a precaution. This posed an imediate health and safety, and personal rights risk to the child in care. The facility agrrees to submit documentation as proof of completion to LPA by 10/20/25. 101238(a) · 101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.
Facility will submit proof (copy) of a detailed plan of what steps the facility will take to ensure that the facility is clean and sanitary, in addition to the steps they already take. This plan shall be signed and dated. The facility will also submit proof of (copy of documehts) the recently employed Janitor. All of the above items will be submitted to the Department by the POC due date. Based on observation and interview, the facility had debris on the floor of the preschool classrooms including food, dust, discolored marks on the floor, and brown and black marks on the floor of the restrooms and base of the toilets that children use. Also, there were brown markings and trash on the floor of the kitchen that is utilized for the entire facility.

Director agrees to submit a copy of the training's agenda and sign-in sheet for each teacher that attends. teacher at any time...Supervision shall include visual observation. It was determined that a preschool child's class transitioned into their classroom and one child was left in the breezeway playground. However, the subject child was observed by a nearby school-age teacher for the whole time until the child was walked back to their class. Director also agrees to submit a letter to LPA stating the plan on moving forward from the incident and state all corrective actions that will take place. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Pertinent interviews disclosed that two children were briefly left unsupervised leading to an inappropriate action occurring in the bathroom. During the investigation process, it was revealed that the subject classroom had a temperatue of 58 degrees fahrenheit. This was due to the facility's heating was not working. This took place on 01/09/2024 and 01/10/2024. The facility's heating system was fixed on 01/11/2024.

101238(g)(1) · (g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. Work order and/or pictures of purchase and replacement on the cabinets must be submitted to CCL by deadline. cleaning supplies were observed by LPAs in the lower shelf of the upper cabinet unlocked as teachers arrived and left their purses on shelf upon arrival to the classroom. Classroom: Preschool and Discovery Preschool. This posed a potential health and safety risk to children in care.
