While touring the facility and checking the Facility Personnel Report Summary, LPA observed assistant nurse Ariel Donley was not assoiciate to the facility which poses an immediate health, safety or personal rights risk to persons in care. Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above. While touring the outdoor activity space at 10:26 a.m., LPA's observed four screws on a wooden structure that were not fully intact. The screws were hanging outside the wood approximately 1 inch which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 11/09/2023
