The director will have a staff a meeting and provide supervision training and will have sceniors to practice. The director will provide a list of the signatures of the staff that attended the meeting and will send the proof to the LPA via email by the POC due date. Based on LPA interviews, S1 failed to provide supervision for C1. There was absence of supervision when S1 did not walk over to help C1 get down from the chair which cause C1 to severely injured his back. Lisa Kathleen, new Interim director has provide a care and supervision to staff within the last two months on previous substantiated finding on 08/19/2022. Director agrees to provide a memo to remind all staff of the importance of supervision and procedures for restroom use protocols at the facility. This requirment was not met as evidence by review of video footage on 06/22/2022. Video footage revealed that on 05/23/2022 a child was left unattended in the restroom for at least 4-5 minutes. Statment by Staff #1 and previous director Krista Bartolome coincide that the incident did occur. This poses an immediate risk to the child duriing that time. Lisa Kathleen will also provide this memo communication that was given to all staff to LPA Gutierrez via email @ stella.gutierrez@dss.ca.gov by the specified date on this report of 11/11/2022 by 5:00 PM PST SUPERVISOR'S NAME Patricia Magana

C1 was handled in a rough manner by Staff #1 and
based on interview statements the majority of the staff are restraining the children while in care other known as "holding". This poses an immediate risk to children in care before 09/09/2022 that addresses the discipline policy and positive redirection. ID was also asked to address "holding" with the facility staff. ID will provide LPA with these documents by the end of the business day of 08/24/2022 and documentation showing that the training was provided by 09/09/2022 by 5PM via email stella.gutierrez@dss.ca.gov
