One out of the two pieces has completely worn down, exposing sharp pieces of wood. The water fountain itself works, and has collected dirt and leaves on it, as well as calcium from water. This was not met as evidenced by: During a tour of the center, LPA noted areas of the center not being used with signs of rodents. Director was able to speak with the owner and get things started on the clean up of the off limit areas. Furthermore, director will send in a written understanding of what a clean, safe, sanitary and repaired area looks like. Additionally observed were cobwebs, dust, debris, rugs, large toys, and stacks of paper piled on top of each other. LPA noted a foul odor in one of the on-limits classrooms, & unable to determine its source.

This was not met as evidenced by Director was able to get droppings cleaned up. Furthermore, director will send in a written understanding of what measures they are taking to ensure flies, other insects or rodents are not a threat to the health/safety of the chidlren. During a tour of the center, LPA noted areas of the center not being used with signs of rodents. Pertinent parties stated on/about 08/18/25, staff were instructed to sweep up rat droppings found in a first-floor storage closet and additional droppings on the second floor.LPA reviewed a photo of a large possum that had reportedly left fecal smears inside the facility. This poses a potential health and safety risk to the children in care.

101170(e) · (e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility. This was not met as evidenced by: During a tour of the facility, LPA determined S1 does not have a criminal record clearance. Immediately, director removed this individual from the room, away from the children. Director agrees to communicate with LPA what their plan is regarding S1, moving forward. This poses an immediate threat to the health and safety to the persons in care.
