Owner took all responsibilities away from the staff member 5/13/25. Director stated on 5/19 director and owner will be meeting with each staff member for review of training. Based on the facility self-reporting and staff interview a child was left outside on the playground, alone. This is an immediate health, safety and personal rights risk of children in care. Civil Penalty 421IM was assessed today for $500.00 Director stated she will send the topic and list of participants to CCL 5/19/25 and also a plan of action to avoid this incident in the future. Director immediately took child back to the classroom 4/21/22. Licensee stated she will conduct a training for all staff on CCR 101229 (a) (1), including “Name to Face” procedures and will send a list of participants to Community Care Licensing by 4/27/22. Based on director self-reporting the incident, a child was left unattended on the playground. This is an immediate risk to the health and safety of children in care. Civil penalty LIC421IM assessed today of $500.00. Licensee created a log that each administrator will sign off on the log stating that all children are counted for.
