101161(a) · 101161 Limitations on Capacity (a)A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation.

, as evidenced by Director stated they are working on rectifying the deficiency, and will submit documentation necessary to extend the period for participation in their toddler component license. Director will submit documentation to LPA by 4PM POC due date. Director did not comply with the section cited above as interviews, and documentation revealed that there were children still enrolled over the age of 36 months in a Toddler component classroom. The center did not request extension from the department, and did not obtain documentation necessary to extend the period for participation in the toddler component license, which poses a potential Health, Safety, or Personal Rights risk to persons in care.

101229(a)(1) · 101229 Responsibility for Providing Care and Supervision (a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time...shall include visual observation. Director reviewed strategies with staff regarding active supervision, staff have developed a plan to ensure supervision in their classroom, staff are doing their name to face, and staff have assigned stations in the classroom to monitor doors. Corrections have been made since incident.

Based on interview, and record reviews, Director (D) did not comply with the section cited above as (D) self reported to the dept. and submitted an Unusual Incident Report (UIR) on 10/30/24 that a child left a classroom and walked down the hallway to another classroom without staff supervision for approximately 1 minute, which poses an immediate health, safety, or personal rights risk to persons in care SUPERVISOR'S NAME: Natalie Dunaway
