Assistant director provided documentation to LPA on 6/27/2025 that the facility has implemented a written diaper log as additional step to ensure staff are regularly checking and changing diapers. This deficiency is cleared. Based upon staff interviews, at least 1 child was left in soiled clothing for extended period of time which posed a potential health, safety and personal rights risk to children in care.

Ms. Hann stated that the facility closed on 3/5/2025. Facility re-opened on 3/6/2025 and found out that there was still issue with the pressure. Children were using hand sanitizer and buckets of water to flush the toilet. This issue was temporarily resolved, via bypassing pipe using a industrial hose going into the building. Based on interviews, it was determined that the facility did not have running water on 3/5/2025 & 3/6/2025. Facility closed on 3/5/2025, however the next day 3/6/2025, there was insufficient water pressure for handwashing or to flush the toilet. LPA observed that the toilets flushed and that there was running water for handwashing. Facility is currently working with maintenance personnel to repair the pipe. Moving forward, the facility will close operation if water is not delivering enough pressure.

Ms. Han stated that she will report to the department immediately when an unusual incident occur. She also stated that she will contact Duty Line to determine if incidents are reportable or not. Based on interviews, licensee failed to report to the department that the facility closed down on 3/5/2025 due to a broken pipe resulting to insufficient water being delivered to the building. Facility also did not have sufficient running water part of the day on 3/6/2025.

A ratio of one fully qualified teacher (as specified in Section 101216.1(c)) and one aide for every 18 children in attendance...allowed when the aide meets the qualifications specified in Section 101216.2(d). This was corrected today. Temporary correction was to move 3 youngest in the group to the Teddy Bears room. A fully qualified teacher and an aide (with 6 units) will be placed in the Grizzlies room.

Director removed the structure from the playground and updated the supervision policy with staff. Director will submit written supervision policy with staff signatures via e-mail to LPA Lane by 7/8/21. Based upon facility reports and LPAs interviews with children, staff, parents and Director it was determined that the incident could have been prevented with proper supervision creating an immediate health, safety and personal rights risk to the children in care.
